Online Data Request Form

You can now request your CSN reports online! 
Just fill out the online form and click a button to submit it to Institutional Research.

 How to Use the CSN Data Request Form

1. Click the link (above) to go to the Data Request Form (DRF).
2. The form is an Adobe PDF file. You need Adobe Acrobat Reader to open it. (See below on how to get Adobe Reader.)
3. The form contains fields and checkboxes where you can enter your contact information and various details of your request.
   1. To place your cursor in a field, click on the box or press the Tab key to move through the fields.
   2. To go backwards through the fields, hold the Shift key and press the Tab key.
   3. To check or uncheck a box, click on it or press the Space bar.
4. You can check as many boxes as apply to your request.
5. Enter explanatory details as needed.
6. When you are done, you can:
   1. Save the completed form to your computer (you must use Save As)
   2. Print the completed form with the Print Form button
   3. Submit the form by email with the Submit Form button
   4. Erase the form and start over with the Erase Form button
